This form applies ONLY to IUB Chemistry Personnel. Print out this form and return it to Frank Gao, (room A423A, Chemistry Department), or to your course instructor.

This application serves as your request for an account on the Departmental SGI workstations PCHEM01, PCHEM02, PCHEM03, PCHEM04, PCHEM05, and PCHEM06. To request an account on STEREO2, STEREO3, or SPLATTER, contact Frank Gao. To request an account on a SGI workstation in a research group, contact the research group's SGI system administrator.

Once your accounts have been made, a copy of this application will be returned to you through chemistry mail, or through your course instructor. Information about your new account will be listed at the bottom of this application.

Direct all questions to the MolViz Facility staff. Contact Frank Gao if you need an account earlier than one week from the date of this application.
